Registration for Minor Box lacrosse in Zone 5 has to be done with the association closest to your residence. The OLA has a residency rule, where, a player must be registered to the association closest to where they reside and go to school.

Some associations that border one another, have defined boundary lines. And this rule doesn't apply.

Registration forms are prepared for the 2010 season and will be available in Mid-January.

Required to register
All players - player's signature, parent or guardian's signature, fee
New players - copy of proof of age must accompany registration, health card #.

Proof of age may be provided from :

• Birth Certificate
• Passport
• Baptism certificate
• Photocopy of Health Card (front and back)
• Letter from school, on school letterhead, signed by Principal
• Drivers license (front and back)
